This 7 page document needs to be completed before the first day of practice of your interested sport and uploaded to the Rank One portal. 

**Please note-The Caesar Rodney School District will NO LONGER accept paper copies of physicals.  All registrations and physicals must be submitted online through Rank One.  Please do not email or drop off paper copies.

Physical Upload & Submission

The Caesar Rodney School District has partnered with Rank One as the exclusive online registration platform for all student-athletes interested in playing a sport at any of our schools.

All student-athletes MUST register and upload their DIAA physical forms online through Rank One.
